ASSUMPTION OF RISK
I acknowledge that participation in baseball activities, including but not limited to batting, throwing, warm-ups, competitions, physical activity, and related event activities, involves inherent risks that may result in serious injury, illness, permanent disability, property damage, or death. These risks include, but are not limited to, being struck by baseballs, bats, equipment, contact with participants or spectators, weather conditions, facility conditions, and other known and unknown hazards.
I knowingly and voluntarily assume all risks associated with participation in the Between the Lines Baseball Co. & Los Diablos Athletic Club's Youth Home Run Derby.
RELEASE OF LIABILITY
In consideration for being permitted to participate in this event, I, for myself and/or on behalf of my minor child, hereby release, waive, discharge, and covenant not to sue Between the Lines Baseball Co., its owners, directors, officers, employees, volunteers, sponsors, vendors, facility operators, affiliates, agents, representatives, and event staff (collectively referred to as the "Released Parties") from any and all claims, liabilities, demands, causes of action, damages, losses, costs, or expenses arising out of or relating to participation in the event, including claims arising from ordinary negligence.
MEDICAL AUTHORIZATION
I certify that the participant is physically capable of participating in this event. In the event of an emergency, I authorize event personnel to secure emergency medical treatment for the participant if a parent or guardian cannot be immediately contacted. I understand that all medical expenses incurred are my sole responsibility.
MEDIA RELEASE
I grant Between the Lines Baseball Co. permission to photograph, video record, and otherwise capture the participant's image, likeness, voice, name, and performance during the event. I authorize the use of such materials for promotional, marketing, advertising, social media, website, broadcast, and other lawful business purposes without compensation or additional approval.
CODE OF CONDUCT
Participants, parents, and spectators are expected to conduct themselves in a respectful and sportsmanlike manner. Between the Lines Baseball Co. reserves the right to remove any participant or spectator whose behavior is deemed unsafe, disruptive, abusive, threatening, or detrimental to the event environment.
ACKNOWLEDGMENT
I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY IN ITS ENTIRETY. I UNDERSTAND ITS TERMS AND ACKNOWLEDGE THAT I AM GIVING UP CERTAIN LEGAL RIGHTS. I SIGN THIS AGREEMENT FREELY AND VOLUNTARILY.
